The Lead Social Worker supports commercial integrated care programs by supporting patients and care teams to address needs, and providing subject matter expertise to model of care and operations teams.
Essential Duties & Responsibilities
1. Patient & Care Team Direct Support
- Case consultation with IKC RNs and ROMs for Advance Care Planning, End of Life / Supportive Care patient needs, behavioral health, and/or other social work needs. Support development and execution of patient centered care plan based on identified patient needs.
- Provide direct patient support as needed for escalated cases in licensed states
- Support complex case reviews as part of multi-disciplinary team to guide patient care planning
- Lead supportive care case reviews to guide care planning for patients at end of life
- Engage in care plan execution for highly complex patients, including direct patient engagement (where licensed), complex care navigation needs, clinic / provider collaboration
2. Model of Care Subject Matter Expert (SME)
- Serve as subject matter expert (SME) resource for advance care planning, supportive care, behavioral health, and other social work related model of care and continuous improvement projects
3. Care Team Subject Matter Expert (SME) & Quality Oversight
- Serve as SME for advance care planning & supportive care programs to review cases / calls, identify BDPs and opportunities for improvement, support teammate coaching
- Serve as SME for teammate training
4. Other duties as assigned
Required Education, Experience, and Qualifications
- 8-10 years social work experience including demonstrated experience and expertise supporting advance care planning and end of life / supportive care needs required
- Additional demonstrated experience in renal care and with behavioral health support strongly preferred
- Masters' degree in social work (MSW) required
- Current licensure in state of residence, willingness to obtain additional licensure required
- Intermediate level skill in MS Word, Excel, PowerPoint, and Outlook
