1. Responsibilities
a. Elicit and record physical and mental health status, psychosocial history, including review of bodily systems.
b. Performs examinations such as physical, sick visits, etc. and records findings of such. This includes performing emergent evaluations as necessary.
c. Initiates appropriate diagnostic tests to screen or evaluate the care-recipient's current health status.
d. Assesses findings of history, review of systems, physical examination and diagnostic tests and formulate a diagnosis prior to implementing a treatment regimen.
e. Perform diagnostic testing within the office settings as indicated.
f. Combines and adapts data to determine that preliminary diagnosis and therapeutic plan utilize principles of prevention.
g. Identify health problems and learning needs of the care recipient.
h. Plan, teach, promote and manages physical and mental health care in a continuous program.
i. Implement and manage treatment regimens and administer, prescribe, dispense and procure pharmacological agents.
j. Administers injections, immunizations and oral medications.
k. Arrange appropriate referrals.
l. Initiate appropriate emergency treatment in life-threatening or unusual situations in order to stabilize the care recipient.
m. Triages patient telephone calls. This includes providing telephone consultation and follow-up phone calls.
n. Continually monitors and ensures that continuity of care is uninterrupted between physician/nurse practitioner visits.
o. Provides appropriate care for nursing home patients as indicated.
p. Performs privileged procedures safely and with informed consent, as indicated. Documents all patient care activities accurately and in a timely manner as per policy and procedure and according to regulatory mandates.

2. Professional Development - Ensures improvement in practice setting by assuming responsibility for self-development and life-long learning. Provides guidance to others in the practice.
. Provides mentorship to the office clinical staff in policies and procedures that impact the care of patients and promotes clinical competence and the acquisition of new skills for clinical staff.
a. Collaborates in the development and implementation of educational plans for identified learning needs of clinical colleagues.

3. Press-Ganey
. Works effectively and cooperatively with other members of the team to identify opportunities for improvement in Press-Ganey results and actively participates in recommending plans of action to address any identified issues.

4. Doctors' Health Partnership
. Works effectively and cooperatively with other members of the team to identify opportunities for improving processes related to the management of the Doctors' Health partnership. Actively participates in recommending plans of action to address any identified issues.

5. Other Duties
. Provides other functions common to the Physician Assistant role for which the PA-C is educationally and experientially prepared.

Qualifications
Experience Minimum Required
• Patient Care ranging from newborn to geriatric.

Education Minimum Required
• Successful completion of a physician assistant training program approved by the Committee on Allied Health Education and Accreditation (CAHEA).
• Additional education may be required for some specialty areas.
• Successful completion of the prescription privileges exam administered by the NH Board of Pharmacy for PA's practicing in NH.

Special Skills Minimum Required
• Current licensure by the New Hampshire Board of Registration of Medicine; Registration with DEA.

Licensure and/or Certifications Required
• PA Licensed
