Position Summary

The Patient Navigator act as a care coordinator, facilitating communication between the high-risk patients, their families, providers and other members of the healthcare program. The main focus is to offer individualized assistance to patients and families, in order to help overcome health care system and community barriers and help facilitate consistent and timely medical care, from the initial hospitalization, through all phases of the outpatient process.

Essential Duties and Responsibilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned.

  • Assists with scheduling medical and specialty appointments, as well as provide reminder phone calls for various scheduled appointments and/or follow-up calls after scheduled appointments
  • Help patients connect with transportation resources and give appointment reminders
  • Consistent chart documentation and abstraction to ensure effective communication and continuity of care
  • Provide referrals for services to community agencies as appropriate
  • Work closely with the Care Navigation team to help ensure that patients have comprehensive and coordinated care that aligns with the patient-centered care plan
  • Be knowledgeable about community resources appropriate to the needs of patients/families
  • Assists in the collection and assembly of quality improvement information, for the purposes of tracking and trending
  • Assists patients with adherence to existing self-management goals or development of new goals (in collaboration with Clinical Providers)
  • Assist in identifying new needs with current patients or identifying when the plan of care is not effective. Discuss or schedule with the Nurse care manager to move towards new plan
  • Performs other duties as requested
Qualifications

Education
  • High school graduation is necessary; Associate's degree or higher is preferred.
Experience
  • At least 2 years of experience in healthcare, case management, and or/patient-oriented position.
  • At least 2 years of experience in providing customer service in a patient care environment.
  • Experience with diverse communities is preferred.
  • General working knowledge in the areas of patient care, health, and wellness.
