JOB SUMMARY

The Nutrition Transplant Specialist researches nutritional requirements of patients with complicated medical conditions and develops care plans. This position provides nutrition therapy and education to patients, family members, and community.

ESSENTIAL FUNCTIONS OF THE ROLE

Performs nutritional assessment of patients with complicated medical conditions using advanced biochemical, clinical and anthropometric data. Analyzes patient medical and non medical information through interviewing patients and care givers, and reviewing medical records interpreting and applying results.

Assesses calorie, protein, fluid, vitamin, mineral, insulin and electrolyte needs. Formulates and individualizes intravenous and enteral nutrition therapy with macro and micronutrient manipulation. Determines total parental nutrition therapy rates and duration of infusion. Monitors biochemical indices, recommends laboratory test and evaluates intolerance and causes. Develops tube feeding orders and protocols, and prescribes tube feeding formulations. Develops care plans.

Provides individual and group nutrition counseling on complex diets requiring a combination of restrictions. Assesses factors affecting dietary compliance; socio economic factors, past history, family dynamics, etc. Determines and implements dietary plans. Provides information on fitness and exercise as related to weight management and diet. Documents pertinent information in appropriate medical record, and monitors patient to determine need for appropriate modification in nutritional management.

Participates in multi-disciplinary patient rounds, case conferences and discharge planning in order to gather and exchange information. Surveys patient response to care plans to maximize the efficiency and effectiveness of patient care. Participates in various meetings related to nutrition education, patient care standards, or dietetic internship programs in order to gather and exchange information.

Researches, develops, and delivers educational and instructional programs and classes for patients, families, staff, dietetic interns, community groups and other interested healthcare professionals. Develops nutrition educational tools such as handouts, pamphlets, booklets, etc. Documents training programs for staff. Assists with training, instruction and evaluation of dietetic interns. Develops nutrition classes, educational materials and curriculum for dietetic internship program.

Participates in research studies through data collection and literature reviews or as co-investigator. Develops and delivers presentations on research related topics. Evaluates and appropriately utilizes pertinent current research information related to nutritional care.

Participates in quality assurance programs by assisting in the development of patient care criteria and analyzing actual care delivered.

QUALIFICATIONS

- EDUCATION - Masters'

- EXPERIENCE - 2 Years of Experience

- CERTIFICATION/LICENSE/REGISTRATION -
       Registered Dietitians (RD)

Must have Master’s degree and be a Registered and Licensed Dietitian. At least 2 years experience is required. Optional but beneficial certification: CNSC, CDESE, CCTD
