Description:
The Judge Group is looking forNurse Practitionersto performin-home health assessments on Medicare Advantage Members in Linn County, OR with one of the largest healthcare providers.
• Pay- HIGHLY competitive, paid per assessment.
• Contract- ongoing (no end date)
• Schedule - Flexible schedules. Ability to work any day of the week from 7AM - 8PM. (can be made around another current position). At least 30 assessments per month are required.
• All training and equipment are provided by the client.
• 1099 position.
• Mileage reimbursement from assessment to assessment.
• Travel reimbursement for hotel lodging (when applicable).
• A mix of in-home and virtual assessments.
Overview:
The provider will review and observe a member's current health, medical history, medication adherence, social environment, and other risks in about a 1-hour non-invasive in-home visit (non-prescribing, non-treating). This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits.
Responsibilities
• Conduct Health Risk Assessments (HRA) on insured plan patients in their homes or plan designated areas.
• Identify all active and chronic disease conditions and determine all physical, mental, and social needs present at the visit.
• Obtain vital signs, measure BMI, review pharmacological therapy and conduct a physical examination.
• Conduct necessary screenings to capture HEDIS measures and close gaps in care.
• Educate and counsel patients and family on any conditions identified during assessment.
Qualifications:
• Have a Minimum of 1 year of experience as a Nurse Practitioner.
• Master's Degree required OR commensurate experience and satisfactory completion of NP licensure.
• Current unencumbered RN and NP licensure in state of OR.
• Board certified by the AANP, ANCC in an approved specialty.
• Board Certifications accepted:
• Acute Care
• Adult Health
• Family Health
• Have a current BLS, ACLS or CPR certification.
• Have strong computer skills and familiarity with Employee health/medical record software.
