Job Summary:
Provide acute and primary care services in the inpatient setting.
As a nurse practitioner on the inpatient unit, the role of the nurse practitioner will act as a care provider with the primary physician team in the care of the child with complex medical needs.
• Enhance health outcomes and improve the quality of care for children with complex health care needs through coordinated care and increased accessibility.
• Cohesive coordination of care including patient's medical history, medications, nutrition, medical interventions (trach, vent, airway clearance, O2 parameter, O2 titration), specialist/follow up plan, goals, therapy recommendations (AFOS, safety adaptive device), red flags, and other areas of needs including developmental, educational, social, and financial concerns to achieve optimal health for children with complex medical needs. A liaison helping bridge parents' long term and current comfort care goals with medical management
• APRN follow up telehealth appointment after discharge (within 7 days of discharge to facilitate transition to home team)
• Participate in family-centered rounds.
• Implementation of care conferences with families and the entire medical team to help understand barriers to taking the child home, goals of care, and long-term planning
• Increase accountability between team members and families to ensure timely accomplishment of educational goals
• Care plan to act as the main form of discharge instructions. The document will serve as a guide for families and other providers caring for the child.
• Home medications are reviewed periodically to ensure the correct medication profile at the time of discharge.
• Decrease unplanned transports back to referring hospital.
• Expedite and improve the discharge process
