Merakey, Bethlehem is currently seeking a LPN Blended Case Manager.

Under the direction of the Blended Case Management Supervisor, this position will be responsible for coordinating and implementing services for consumers as identified in the service plan. Core responsibilities will include coordinating the overall medical care for consumers, following through with Doctor's medication orders, oversight of all medication management, providing supportive counseling, training, and case management services utilizing a flexible schedule, and will include on-call responsibilities.

• Demonstrate a Rehabilitation/Recovery attitude toward service delivery by providing consumer-driven strength-based approach to medical services, case management and crisis services.

• Assist consumers in identifying and accessing medical needs through regular assessment, and coordinating and assessing necessary medical services.

• Assist Primary Case Managers in gaining necessary information related to the consumers' medical health and services available.

• Coordinate service delivery, both internal and external, in relation to medical services.

• Complete initial nursing evaluations, including medical history summary and yearly updates on each consumer that is monitored, as needed

• Provide medication monitoring, education and recording as needed.

• Work closely with primary case workers and psychiatrist(s) to follow through with MD orders.

• Assess clients for symptoms and side effects of medication and provide education, management, and documentation as needed.

• Educate and train staff, family members, and community providers in relation to all areas of medical treatment and its relationship to psychiatric treatment.

• Maintain pharmaceutical supplies, and other treatment supplies as required for assessment and treatment.

• Participate in service delivery for all the consumers on the team.

• Participate in service plan meetings, treatment meetings with other providers, and team clinics, as indicated by Team Leader. Follow through with clinical decisions as determined utilizing a team approach to problem solving.

• Act as primary case manager when needed by assisting consumers in obtaining and maintaining basic living needs and skills related to housing, food, psychiatric and medical care, transportation, socialization, education and employment, in a manner that is clinically appropriate.

• Attend and participate in all meetings, supervisions and training in a professional manner.

• Act as a liaison with family, service providers, and community supports and services, as needed and appropriate.

• Maintain professionalism as per corporate policy during all aspects of service delivery.

• Fulfill on-call duties as assigned.

Complete all paperwork in an accurate and timely manner.

Must be a Registered Nurse, Licensed Practical Nurse or have a bachelor's degree in nursing with a minimum of 12 credit hours in social science and a minimum of 2 years of experience in a mental health setting.

Valid PA Drivers License and other appropriate state and federal clearances.
