Location: Dallas, TX
Salary: commensurate hourly rate
Description: Our client is currently seeking an LPN for their urgent care in Dallas, TX.

Specific Duties:

• Works with the providers and clinic staff to identify patients with chronic disease diagnoses.
• Supports longitudinal care of the patient with stable chronic care conditions
• Supports transition of the patient with stable chronic care conditions from inpatient to outpatient setting
• Notifies providers and/or supervising RN of changes to patient's health condition
• Performs accurate and timely documentation in the electronic medical record
• May perform clinical tasks within their scope of practice

Must have qualifications/experience:

• Licensed Practical/Vocational Nurse with an active and unrestricted license to practice
• Current BLS certification
• Two or more years of experience in a physician's office, clinical, or hospital setting
• Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
• Excellent verbal and written skills
• Strong interpersonal skills
• Ability to interact productively with individuals and with multidisciplinary teams
• Proficient computer skills to work efficiently with electronic medical records
• Excellent organizational and prioritization skills
• Must be COVID vaccinated (with proof)

Shift Start/End Time: 7:45 AM-5:00PM (M-F)
