Position Summary
The Licensed Clinical Social Worker (LCSW) is responsible for providing psychosocial and clinical support services to patient and their families in the outpatient setting. The LCSW assesses patients from a culturally and ethnically diverse population and provides outpatient clinical intervention while establishing plans for effective management of identified needs. The LCSW acts as an advocate for patients and provides supportive, short-term counseling and crisis intervention to individuals and/or their families, as well as facilitating their transition to longer -term therapy with other providers as needed. The LCSW also develops and facilitates ongoing psycho-educational support groups and programs. This is a dual role also supporting DGIM.
The DGIM Social Worker (SW) is responsible for providing psychosocial clinical and resource support services to patients and their families in the outpatient setting. The SW assesses patients from a culturally and ethnically diverse population, establishes plans for effective management of identified needs by providing resource referrals and coordination, and provides outpatient clinical intervention and care when needed. The SW works closely with other healthcare providers and members of the interdisciplinary healthcare team to provide services to patients in the Division of General Internal Medicine and the Division of Geriatrics and Palliative Care. The direct clinical component of the position is expected to encompass approximately half of total time. The SW acts as an advocate for patients and provides supportive, short-term counseling and crisis intervention to individuals and/or their families, as well as facilitating their transition to longer -term therapy with other providers as needed.
