Position Summary

The Licensed Clinical Social Worker (LCSW) is responsible for providing psychosocial and clinical support services to patient and their families in the outpatient setting. The LCSW assesses patients from a culturally and ethnically diverse population and provides outpatient clinical intervention while establishing plans for effective management of identified needs. The LCSW acts as an advocate for patients and provides supportive, short-term counseling and crisis intervention to individuals and/or their families, as well as facilitating their transition to longer -term therapy with other providers as needed. The LCSW also develops and facilitates ongoing psycho-educational support groups and programs. This is a dual role also supporting DGIM.

The DGIM Social Worker (SW) is responsible for providing psychosocial clinical and resource support services to patients and their families in the outpatient setting. The SW assesses patients from a culturally and ethnically diverse population, establishes plans for effective management of identified needs by providing resource referrals and coordination, and provides outpatient clinical intervention and care when needed. The SW works closely with other healthcare providers and members of the interdisciplinary healthcare team to provide services to patients in the Division of General Internal Medicine and the Division of Geriatrics and Palliative Care. The direct clinical component of the position is expected to encompass approximately half of total time. The SW acts as an advocate for patients and provides supportive, short-term counseling and crisis intervention to individuals and/or their families, as well as facilitating their transition to longer -term therapy with other providers as needed.

Essential Duties and Responsibilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned.

  • Conducts initial and ongoing comprehensive psychosocial assessments and develops a care plan for and with DGIM patients and their family members to enhance, maintain and promote optimal psychosocial functioning. .
  • Provides patients and families with mental health counseling and therapy, crisis intervention counseling, and grief counseling.
  • Facilitates the transition of patients in need of long-term therapy to other mental health providers in the community.
  • Maintains comprehensive, timely and accurate documentation of patient assessments and interventions in the electronic health record.
  • Identifies, assesses, and addresses the psychosocial needs of patients and their family members; advocates for better meeting of those needs with appropriate agencies or with further program development.
  • Provides patients and families with long-term planning consultation, information and referrals to appropriate community resources, financial and/or healthcare resources.
  • Collaborates with representatives of community services to enable patients to resolve non-medical problems and to facilitate referral of patients to other sources of assistance.
  • Provides referrals for community resources that may include but are not limited to the following: Housing assistance, Transportation, Nutritional programs, Senior Centers, Wellness Centers & Adult Day Care, Legal Services, Health Insurance Information, Case management/Geriatric care management, Private Home care / home health, Financial assistance, Prescription assistance, Emergency services (i.e. shelter, food, etc.), Mental Health, Substance Abuse, Domestic Violence, Support Groups / Counseling, Dental / Vision Resources, Emergency Response Systems (i.e. LifeAlert), Advanced Directives, Hospice, Caregiver supports, POA/Guardianship, Educational and supportive materials.
  • Provides patient navigation services by providing resources for various patient assistance programs, including but not limited to financial, housing, Medicaid enrollment, free or reduced cost medication, Supplemental Nutrition Assistance Program, social security benefits, disability benefits, copayment assistance and insurance coverage.
  • Coordinates with patients and their families to resolve obstacles to care and decrease no shows.
  • Coordinates with home health agencies and care providers to provide optimum care.
  • Maintains and organizes fundraising for DGIM/Geriatrics Patient Assistance Fund.
Qualifications

Education
  • Masters Degree in Social Work and current licensure in clinical social work (LCSW).
Licensure
  • A current social work license in the District of Columbia.
Experience
  • Minimum of 2 years of clinical social work experience. Knowledge of mental health resources available in Washington, DC preferred.
  • Knowledge of psychosocial resources available in Washington, DC preferred.
