We are seeking a Licensed Practical Nurse to join our team in our Wilmington, DE 19805 location.

Job Description:

The physical health professional navigator (PHN) is a Licensed practical Nurse (LPN). As such, this staff member is uniquely trained to consider the interplay of the person, environment, and health (physical and behavioral). The primary role of the Nurse Navigator is to function as a health advocate to aid individuals in receiving the physical health and behavioral health care then need, in addition to assisting in coordinating medical and behavioral health appointments, hospital care and discharge to the community, arranging travel to appointments, and the development of personal plans for consumer health collaboration and wellness.

Earn $30/hr
