The Lead Care Manager will oversee the operational and programmatic care management service delivery for specialized areas. Collaborates with nursing staff, allied health, admitting and physicians in program development. Acts as a resource/clinical expert to ensure high quality patient care standards are met and maintained. Acts in the absence of the Director, Inpatient Care Coordination and Resource Utilization. Actively participates in the utilization review process and data analysis to ensure optimal patient outcomes.

Principal Responsibilities
The Lead Care Manager:

  1. Provides assistance with managing the daily operations for inpatient care management.
  2. Assists with performance evaluation of Inpatient Care Management Assistance staff, assessing and developing program annual goals.
  3. Provides coverage for the Director, Inpatient Care Coordination and Resource Utilization when away from the facility.
  4. Assists with assessment of programming needs and develops site programming based on the assessment.
  5. Assists with development and implementation of documentation standards.
  6. Monitors personnel staff compliance with policies/procedures and documentation standards ( i.e. emergency procedures, admission/discharge)
  7. Serves as liaison between nursing staff, medical staff, care managers, psychology, and allied health staff for clinical issues.
  8. Provides LCSW supervision to department candidates as needed.
  9. Assists with strategic planning for budgetary needs to support programming goals.
  10. Assists with mentorship and development of new department staff.
  11. Ensures programmatic evaluation and treatment processes are utilized and implemented as indicated.
  12. Assists with interviewing, hiring and orientation of staff.
  13. As a working manager, performs clinical functions approximately 60% of the time and administrative/leadership functions 40% of the time, based on patient needs.
  14. In conjunction with the Director, ensures compliance to regulatory requirements and prepares department for surveys.
  15. Provides weekend on-call coverage as needed.
  16. Performs all other duties that may be assigned in the best interest of the Shirley Ryan AbilityLab.
