Description:
Judge Healthcare is seeking a Home Health RN to cover territory in Montgomery County or Philadelphia.
This opportunity is Full Time & Permanent!
Monday through Friday day shifts with rotating weekends
Home Visits Required
Job Description:
- Manage and provide primary patient care to a select group of patients
- Assure quality and maintain open communication with patients
- Present the patient at interdisciplinary team meetings
- Collaborate with team members to assure patient care delivery needs are met
- Interact with any and all members of the care group both internal and external to the organization as needed
Qualifications:
- PA Registered Nurse License
- Bachelor's Degree in Nursing required
- Home Health / Med Surg experience required
