Job Summary:

The Home Health centralized Weekend Supervisor RN is responsible for clinical supervision and support of all clinicians over the weekend. This position will also coordinate the referral process to maximize conversion to admission time and according to home health regulations and complete necessary workflow for Transfer to Inpatient Facility/Resumption of Care and PRN visits.

This position works under limited supervision and requires independent decision-making, critical thinking, and problem-solving skills based on a sound knowledge of the centralized intake process, home health regulations, internal/external sales, and customer service.

The position will work from the Central Business Office and require in-office time as well as on-call availability from Friday at noon through Monday at 8 am.

Qualifications:

  • RN Required
  • Home Health Experience required
  • Centralized Home Health Intake structure experience preferred
  • HomeCare HomeBase experience preferred
  • Previous experience as Clinical Supervisor, Director of Clinical Services or Administrator in Home Health preferred
  • Coding and OASIS experience preferred
  • Knowledge of, or ability to learn, reimbursement program from payor sources
  • Understanding of Home Health environment including overall Admissions Process
  • Advanced computer skills including Microsoft Office and aptitude to learn company-specific systems
  • Must be able to maintain confidentiality regarding patient, employee, and company proprietary information
Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels

Essential Functions:

Complete all workflow-related tasks through scheduling for weekend SOC/ROCs and other visits as needed.
  • Follow existing referrals with the date of requested evaluation through Monday to increase weekend admission conversion and timely initiation of patient care.
  • Collaborate with the On-call Intake Coordinator to establish appropriate payor source and documentation needed for an acceptable referral.
  • Collaborates with the Director of Business Development for HCBS and agency BDS to ensure the agency meets home health regulations and meets/exceeds referral conversion metrics.
  • Contacts individual agency Administrator/Supervising Nurse as needed for direction on admission, clinical, and staffing concerns.
  • Provides a clinical review for all incoming referrals to determine the appropriate level of care and clinical needs
  • Ensures timely referral scheduling and communication to branch and BDS regarding pending admission.
  • Completes Transfer to Inpatient workflow in HCHB when notified of patient hospitalization
  • Completes Resumption of Care workflow in HCHB when notified of patient discharge from the facility.
  • Schedules Admissions and Resumption of Care visit for the weekend to “On-Call RN” for each agency and communicates with staff to verify availability
  • Communicates with ”On Call Administrator” for each agency regarding staffing concerns and PRN RN availability
  • Completes the Non-Admit process for declined referrals due to not in-network or patient refusal.
  • Conducts daily calls with agency BDS team for referral updates
  • Tracks completion of weekend SOC/ROC visits through Agent Summary Reports
  • Contacts patient to verify scheduling of ROC visit
  • Manages the admission process professionally, timely, and with appropriate follow-up.
  • Provides follow-up reporting to agencies on weekend admission conversion.
  • Provides follow up calls to physician/NP for verbal orders on Mondays
  • Maintains a close working relationship with internal key stakeholders to assure appropriate and efficient care transitions
  • Coordinates with intake weekend on-call team to ensure timely coordination of care.
  • Coordinates with intake weekend on-call team to ensure new patient referrals are retrieved from intake portals, faxes, phone, and email, and entered in-home health EMR timely and accurately.
  • Works in conjunction with home health agency Administrators in evaluating referral appropriateness; including but not limited to written documentation of skilled need, homebound status, current home health orders, and face-to-face encounters as required.
  • Collaborates with Central Intake Department workflow processes to ensure accepted referrals are transitioned from intake to insurance verification/authorization and to scheduling timely and accurately in order to prevent delays in inpatient admission to service.
  • Works directly with Business Development to obtain additional documentation and coordination as needed to transition referrals to admissions in a timely manner.
  • May provide backup clinical phone management to the weekend triage nurse.
  • Responds promptly and courteously to all patient inquiries or problems.
  • Assists Central Intake Department associates with referral entry, workflow, and coordination of referrals as needed.
  • Adherence to all company policies and procedures.
  • Adherence to and compliance with information systems security and integrity.
  • Works to facilitate individual and team development that drives positive results
