The Health & Wellness Coach (HWC) will facilitate, develop, monitor, measure, and report on health, wellness, and chronic disease prevention programs that complement the Everside Health Improvement and Wellness population management programs of the Everside Health participant population. The HWC will present information on how to improve health, prevent disease, and modify health behaviors to individual patients through one-on-one health coaching, as well as to client groups. The HWC will also coordinate efforts among client vendors as needed, to provide cohesiveness of services and to better engage the client population. They will also be responsible for creating and writing educational materials, conducting meetings, and planning informative, interesting, and effective programs.

Essential Functions:

  • Provide health coaching on-site for the Health Assessment Services team at various employer-based locations throughout the US. Will provide immediate coaching for participants for their health improvement and risk factors as indicated by individual health risk assessment data.
  • Develop, implement, and promote health, wellness, and chronic disease programs that emphasize the importance of healthy lifestyles. Some of these program topics will include, but are not limited to Tobacco Cessation, Diabetes Management, Hypertension Management, COPD Management, Cardiac Management, Exercise and Physical Activity, Weight Management, and Weight Maintenance. The programs will include step-by-step implementation plans, program details, resources needed, goals for success, and all tools needed to successfully complete the program
  • Provide health coaching in both individual and group settings
  • Plan and/or assist with client wellness fairs including speakers, health-related demonstrations, displays, and video materials provided by local agencies, insurance carriers, local/regional agencies, hospitals, and other resources
  • Measure, monitor, and report on efficacy of all health, wellness, and disease management programs
  • Review client HRA information, as applicable, and consult on wellness information, compliance program design, and effectiveness of wellness programs
  • Participate in education sessions coordinated by Everside Health clients for their respective, eligible employees
  • Consult on wellness and disease management topics at the request of other benefit consultants, brokers and others in the wellness industry, as needed
  • Provide information on available community health agencies, hotlines for drug/alcohol abuse, abuse, and crisis intervention to be available to and used by the on-site clinic providers in their contacts with patients
  • Coordinate efforts with other vendors (carriers, EAP, etc.) to inform client population of all available resources
  • Contribute to and maintain the Everside database to stay current on new, innovative techniques to promote health, wellness, and chronic disease prevention and management
  • Perform other duties as assigned

Essential Physical Demands:
  • Ability to sit at desk in office location each normal work day
  • Ability to use personal computer
  • Ability to communicate clearly and work effectively in person and by telephone
  • Ability to travel commercially by common carrier
  • Ability to work well in a fast-paced environment, while managing multiple priorities and meeting deadlines
  • Ability to drive to and from client and vendor sites, as needed
  • Ability to lift and pull 25lbs
  • Ability to travel 50-75% of the time

Essential Environmental Demands:
  • Ability to work in climate-controlled office environment
  • Ability to conduct client meetings, on occasion, in a variety of environments

Qualifications:
  • Bachelor's degree from an accredited institution in health education, health promotion, exercise science, nutrition, psychology, or other health-related subject and at least 3 years of experience in developing, monitoring, measuring, communicating, and reporting on health- related topics and programs OR
  • Master's degree from an accredited institution in health education, health promotion, exercise science, nutrition, psychology, or other health-related subject and at least 1 year of experience in developing, monitoring, measuring, communicating, and reporting on health- related topics and programs
  • Completion certificate of National Consortium for Credentialing Health and Wellness Coaches (NCCHWC) accredited Health/Wellness Coach Program or within 1 year of hire date. Eligible to sit for the National Board-Certified Health and Wellness Coach
  • Certified Health Education Specialist strongly preferred
  • Knowledge of HIPPA compliance and regulations
  • Experience creating wellness program materials, particularly in employer-based settings
  • Experience working as a health and wellness coach or integrating wellness/fitness initiatives into the culture of an organization
  • Demonstrated competence in multi-tasking
  • Experience leading and developing presentations. High energy personal style and aptitude for process-oriented thinking
  • Self-motivated
  • Track record of consistent, stable employment along with the ability to work independently and as a part of a team
  • Demonstrated passion for health and wellness
  • Demonstrated proficiency in use of Microsoft Office products

  • High visibility and communication with employees and spouses (if applicable) of organization
  • High visibility and communication with clients, prospects, vendors, and carriers
  • Internal contact with Wellness, Client Services, Clinic Operations, Clinic and Support staff
