Overview
Grant Street Medicine Program is a Grant Funded position at our Pottstown Hospital. This position will be working in the community with wellness and aiding health-care to the homeless population. Responsibilities include overseeing the Administration and going out with the team.
Responsibilities
The Program Coordinator (PC) is responsible for the day-to-day administration of the Pottstown Street Medicine program. The PC assists the Department Manager and Street Medicine Medical Director with administrative tasks as well as provides direct patient support for program participants. This role functions as a liaison between clinical providers, community partners and volunteers. The PC will manage all Pottstown Street Medicine grants and ensure deliverables and goals are met. In addition, the PC will assist patients in accessing health care by coordinating appointments, developing care plans, assisting with completion of applications, etc. The PC will be required to accompany the Pottstown Street Medicine team to off campus sites to provide administrative and patient support.
Hours: Daytime (Flexible with Start)/Some evening work based on department needs/ No weekends/Holidays
Hybrid Position- Work from home 1-2 days a week
