Matrix Providers is hiring a Pharmacist to join our team of talented professionals who provide healthcare services to our Military Service Members and their families at BHC Boone (Med Home Port) in Virginia Beach, VA.

  • Employment Status: Full-time
  • Compensation: This is a salaried position
  • Schedule: Services shall be provided Monday through Friday, between the hours of 7:00 a.m. and 7:00 p.m. The clinic is open on the following Federal Holidays: Martin Luther King Jr's Birthday, President's Day, Columbus Day, and Veteran's Day. Each healthcare worker will be scheduled for a shift length that can range from four (4) hours to twelve (12) hours in length. Any shift more significant than six (6) hours will include an uncompensated thirty (30) minute meal break. In no instance will a healthcare worker exceed 80 hours per two-week period.

Requirements: The Pharmacistmust have and meet the following:
  • Degree: Bachelor's degree in pharmacy.
  • Education: Graduate from a college or university accredited by the Accreditation
    Council on Pharmaceutical Education (ACPE) approved continuing education of at least 15 hours which maintains pharmacy skills and knowledge in the preceding two (2) years.
  • Experience:
    • Possess experience as a pharmacist for 12 months within the preceding 24 months.
    • Possess two (2) years of clinical practice managing and modifying drug therapy
      according to written protocols or collaborative practice agreements between the pharmacist and individual physician who are responsible for the patient's care or patient-centered medical home experience within the preceding three (3) years.
    • Have held clinical privileges as a clinical pharmacist within the preceding thirty-six month (36) months in a military treatment facility.
    • Possess experience in the use of pharmacy computer systems such as the Navy
      TRIPHARM Computer System, the Composite Health Care System (CHCS) or retail pharmacy computer systems.
    • Possess one of the following:
      • Completion of an accredited Pharmacy Residency Program.
      • Possess board certification (from the Board of Pharmacy Specialties) in Pharmacotherapy or Ambulatory Care Pharmacy or Psychiatric Pharmacy.
      • Master's degree in Medication Therapy Management (MTM)
  • Licensure: Current, full, active, and unrestricted license to practice as a pharmacist.
  • BLS - Possess and maintain current certification in one of the following: American Heart Association Basic Life Support (BLS) or American Red Cross Basic Life Support (BLS) for Healthcare Providers or American Heart Association or American Red Cross Healthcare Provider Course.
  • Recommendation letters- Provide two (2) letters of recommendation written within the last two (2) years attesting to the healthcare worker's clinical skills. The letters must be from practicing Pharmacists who have personally observed and evaluated the healthcare worker and can attest to the healthcare worker's clinical skills in the care of patients. Reference letters shall attest to the healthcare worker's communication skills, ability to relate to patients, and professional and other interpersonal skills among staff members. They must include the name, title, phone number, date of reference, address, and signature of the individual providing the reference.
Job Summary:
  • The primary workload is the result of consultation requests submitted to the Medical Home Port (MHP) team by staff practitioners. The healthcare worker is responsible for the maintenance and delivery of comprehensive pharmacist services within the personnel and equipment capabilities of the facility and for the quality and timeliness of record preparation and reports indicated to document care provided and procedures performed.
  • Technically direct Government personnel during the performance of pharmaceutical
    procedures. Such direction and interaction will adhere to government and professional clinical
    standards and accepted protocol. The healthcare worker will be subject to guidelines outlined in the command's quality assurance and risk management instructions. The HCW shall perform limited administrative duties as the Commander or designated representative prescribes.
  • Support MHP teams with suggestions for improvements to population health measures. Collaborate with the Home Port Team to monitor Healthcare Effectiveness Data and Information Set (HEDIS) measures and intervene in patients with chronic disease states with
    scheduled visits or educational classes to include (but are not limited to): Asthma, Diabetes,
    Hypertension, Lipid and Pain management.
  • Ensure the quality and timeliness of records, reports, and service documentation. Provide clinical pharmacy direction for all clinical activities within the MTF.
  • Promote a team-based model of pharmaceutical care that results in positive clinical outcomes in a patient-centered Medical Home environment.

  • Perform telephone/email follow-ups to assess new medication regimens as requested by providers.

  • Perform Medication Therapy Management functions to include all five Medication Therapy Management Core Elements.
  • Utilize the Comprehensive Accreditation Manual for Hospitals Joint Commission;
    Manual of the Navy Medical Department, Chapter 21; Navy Pharmacy SOP; hospital instructions; professional journals; and professional society literature. Judgment is required in adapting and applying the established guidelines to the scope of services the pharmacy department provides.
  • Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age-specific dosage, and other pertinent data are appropriate for individual patients. Intervention with providers shall be made for noted discrepancy of above-listed items.
  • Following hospital admission, the Pharmacist may schedule follow-up calls to discuss the medication regimen and answer any remaining questions, providing phone and online support to the patient.
  • Perform a full range of pharmacist procedures
  • Deliver comprehensive pharmaceutical services within the personnel, supply, and equipment capabilities of the facility.
  • Counsel patients regarding the appropriate use of medications.
  • Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
  • Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age-specific dosage, and other pertinent data are appropriate for individual patients. Intervention with providers shall be made for noted discrepancies of the above-listed items.
  • Compound medications as required.
  • Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
  • Supervise inventory control of all pharmaceuticals in the pharmacy, especially scheduled pharmaceuticals, and ensure pharmacy security. Maintain appropriate records and security of all scheduled controlled substances according to applicable regulations.
