Position Summary:Responsible for the supervision of the nurses providing home visits, specific contracts, and unusual occurrence reporting. Responsible for monitoring appropriate job performances, statistical reports, auditing data for compliance of client and employee files. Collaborates with the Director of Licensed Home Care Services Agency and City Home Care Programs regarding overlapping administrative functions.

Principal Responsibilities:

  • Directly supervises the professional staff and oversees the supervision of the paraprofessional staff.
  • Ensures the completion of all paraprofessional supervision as per agency and regulatory/contractual requirements.
  • Ensures that adequate coverage and services are maintained for all clients at all times. Ensures that clients are receiving the appropriate level of care and makes referrals to alternate care (.i.e. CHHA).
  • Monitors all referrals and ensures that cases are accepted and services are provided timely in accordance with agency and regulatory/contractual requirements.
  • Organizes and participates in all contract and CHC audits.
  • Ensures that the clinical staff provides services with Physicians orders and in accordance with the Department of Health regulations.
  • Prepares the On-Call schedule for professional staff and ensures On-Call system is updated and communicated to the Supervising Coordinator on a regular basis.
  • Reviews and analyzes systems and makes recommendations for changes that will enhance the program’s performance.
  • Collaborates with the Director of Licensed Home Care Services Agency and City Home Care Programs and the Manager of Training in the orientation of all coordinator, professional and paraprofessional staff.
  • Makes recommendations to the Director of Licensed Home Care Services Agency and City Home Care Programs for staffing to ensure adequate levels of staff.
  • Monitors the quality of services provided through case review, case conferences, record reviews and report monitoring.
  • Liaisons with other Selfhelp Community Services programs and/or city agencies to comply with contractual agreements including all necessary planning leading to the resolution of payment.
  • Counsels staff appropriately and in a timely fashion in areas concerning deficiency and areas of excellence.
  • Ensures contract standards, communications and compliance of contracts, by timely submission of required documentation to contracts and the payroll department.
  • Ensures that all staff participate in required annual in-services and trainings as per agency, regulatory and contractual requirements.
  • Ensures timely responses to all incidents, complaints and grievances.
  • May represent the Director of Licensed Home Care Services Agency and City Home Care Programs at various meetings as needed.
  • Performs other duties as needed or requested.
  • Adheres to organization’s standards by completing annual mandatory trainings in a timely manner on topics including – but not limited to – Sexual Harassment Prevention Training, IT Security Awareness, Compliance, OSHA, HIPAA and any other training the organization may deem necessary in regards to regulatory compliance or good business practices.
  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
  • Adheres to the organization’s policy in regards to absenteeism and appearance and health and safety standards.


  • Job Competencies & Minimum Qualifications:
  • Graduate of an accredited school of nursing; Bachelor’s degree (B.A.) in Nursing from a four-year college or university preferred
  • Must have current NYS license and registration to practice as a Registered Professional Nurse in New York State
  • Bilingual English/Russian required
  • Computer literate
  • Excellent verbal and written skills
  • Excellent customer service skills
  • Able to multi-task and work independently with a great attention to detail


  • Working Conditions/Physical Demand:
  • Must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.
  • Occasional exposure to toxic or caustic chemicals, outside weather conditions, and risk of exposure to pathogens and unsanitary conditions in client’s homes.
  • Must be able to travel locally by public transportation to make home visits.Valid driver’s license, car registration and insurance as applicable.
