The Home operates two, The Hyde Park and Park Street, Community Service Agencies (CSAs) which serve youth with Serious Emotional Disturbance (SED) who are enrolled in MassHealth Standard or CommonHealth and meet certain medical criteria. This program is part of the Massachusetts Children's Behavioral Health Initiative (CBHI). The two programs serve Dorchester, South Boston, Hyde Park, Roslindale, Mattapan, and West Roxbury.

Our primary goal is to help families strengthen their skills and knowledge in order to move towards their envisioned future.

Reporting to theCentral Intake Manager, the Clinical Intake Coordinator will facilitate and streamline the referral process forthe Community Service Agencies serving Hyde Park and Park Street.

How You Will Be Making a Difference

  • Oversee the intake and discharge process
  • Assess level of care for families on the waitlist making referrals where appropriate to both higher and lower levels of care
  • AttendSystemsof Care meetings as a representative of the program to share current openings and language capacity
  • Works closely with CSA assessor to triage families to Intensive Care Coordination as well as Central Intake to determine appropriate referrals for IHT
  • Develop organizational strategies to manage openings ofCSAteams
  • Participate in clinical coordinator task of supervisor (e.g., on-call, UM, management team, etc.)
  • Foster and strengthen relationship with local DCF area offices, hospitals and BEST to advertise services and obtain referrals
  • Support outreach efforts to increase referrals for both programs
  • Assign cases to staffand monitor the caseload level
  • Complete and manage all reporting to the insurance companies

Qualifications
  • Master's degree in social work, mental health,or related discipline.Knowledge of children with complex psychiatric illnesses
  • Computer literate including Microsoft Office and Internet Explorer with the ability to learn new software applications
