Job Summary

• The Case Manager is responsible to assist in the development, planning, coordination and administration of the activities of Utilization Review.
• Conducts concurrent review of the medical record to determine appropriateness and medical necessity of admission, continued hospital stay, and use of ancillary services.
• Assumes a leadership role within the interdisciplinary team to coordinate services through the continuum of care to achieve optimal clinical and resource outcomes.
• The Case Manager will work with the insurance companies as well as work directly with patients to arrange discharge,etc.
• The Case Manager will work on Med/Surg units as well as ICU, ER and in-patient psych unit- rotating between units
• The work schedule is Monday - Friday 7:30am - 4:00pm mon, as well as every 7th weekend (both Saturday and Sunday), and 1 holiday per year

• 4 year/Bachelor's Degree

Certification and Licensure
