Wellstar Kennestone Hospital - Inpatient Rehabilitation Unit has an opportunity for a Case Manager RN or LMSW

The Case Manager is the primary liaison between the patient, family, the rehab team, outside agencies and their representatives. The Social Worker/case manager provides information about patient's progress, referrals and educational information andadjustment counseling to patients and their families and coordinates the discharge planning process.

Adheres to NASW professional standards, codes of ethics, system and department policies and procedures.

SHIFT- PRN -Schedule will vary; needed for coverage- PTO, Call-outs, leaves, etc.

Required Minimum Education: Associates Degree in Nursing or graduate of an accredited CSWE Program with a master's degree in social work. Licensed in the state of Georgia in respective field.

Required Licensure: Licensed in the State of Georgia in respective field. RN, LMSW or LCSW.

Required Minimum Experience: Minimum of 2 years of experience in medical case management inpatient rehab preferred; Demonstrated counseling skills, knowledge of community resources, federal and local guidelines, funding resources; understanding of adult and geriatric development and age appropriate care; understanding of the effectsof illness and disability and the impact on both patient and family.

Required Minimum Skills: Demonstrates skills or experience in case management. Strong computer skills required. Good communication skills both verbal and written. Excellent time management/organizational skills; working knowledge of the precertification process; Working knowledge of medical terminology and basic computer proficiency required
