OVERVIEW: Peer specialists work as an integrated part of the Care Management team to provide outreach, support engagement, coordinate care, provide advocacy and implement care plans with Health Home members. Peer specialists utilize their own recovery experiences as a consumer of behavioral health, substance abuse treatment and/or chronic medical intervention services to support and empower others. As this is an evolving program, additional responsibilities will be added.

RESPONSIBILITES include but are not limited to:

  • Share and utilize personal experiences as a recipient of health services to engage clients and inform the provision of care management services
  • Engage care management clients to form trusting relationships that foster positive change
  • Educate clients about health homes, care management, enrollment and consents; assist in enrollment and obtaining consents
  • Provide outreach services such as home visits, site visits to service providers and telephone contacts
  • Help members identify and participate a wide range of community-based resources/activities that aid and support their physical and behavioral health as part of their Individualized Care Plan
  • In collaboration with other Care Management team staff, coordinate care with service providers
  • Respond, as needed, to urgent needs and situations in collaboration with Care Management team staff
  • Maintain a working knowledge of available community based services
  • Complete detail-oriented written records of contact with clients in a timely manner utilizing any required electronic record system
  • Participate in briefings, team meetings, and case conferences
  • Attend internal and external trainings as required
  • Maintain client confidentiality
  • Sharing knowledge and experience with other team members to support the team's overall service provision efforts
  • Extensive travel and field work

REQUIREMENTS:

  • Ability and willingness to utilize one's own experiences as a recipient of behavioral health, substance abuse treatment and/or chronic medical intervention services to perform job responsibilities and assist clients
  • Must demonstrate interpersonal communications skills and the ability to empathize with and effectively engage clients
  • Ability to work as part of a service delivery team

EXPERIENCE:

High school diploma or GED

PREFERRED:

Training as a peer specialist or 1 year of work/volunteer experience as a peer specialist is strongly preferred

Experience working in interdisciplinary teams; experience providing care management or care coordination in a medical or behavioral health environment; experience working with the chronically ill

Fluency in a second language such as Spanish, Russian, or Creole
