PURPOSE OF THIS POSITION

The purpose of this position is to develop, organize, and implement the activities of medical surgical patients for BVHS, including, process development, policy development, compliance & billing, staff education, patient intake and management systems, and the development of efficient access initiatives for referrals to the Blanchard Valley Hospital. Navigator will represent BVHS in the community as needed. This would occur in accordance with Joint Commission, Federal, State and local guidelines as well as our established policies and procedures.

JOB DUTIES/RESPONSIBILITIES

During the performance process, please assess the level at which the staff member achieves the job accountabilities. Duty 1: Direct accountability for overall navigation and quality of assigned patients. Assesses current processes and generates new ideas for improvement while seeking ways to maintain productivity. Assumes accountability for identifying problems and issues, generating solutions and anticipating the effects of those decisions.

Duty 2: Develops and leads implementation of assigned patients consistent with the BVHS Strategic Plan. Uses appropriate decision making model that uses planning, researching, resourcing, and implementing in a timely manner in order to develop strategies that will promote growth and financial stability.

Duty 3: Adherence to all relevant BVRHC Administration, Human Resources, Nursing and Department policies and procedures.

Duty 4: Maintains strong working relationships with community BH services.

Duty 5: Community Relations: Identifies needs of the community and develops strategies to meet those needs in balance with the direction of the System. Develops and implements transfer access line for transfers to BVHS.

Duty 6: Budget: Assists in budget and monitors to achieve financial performance goals.

Duty 7: Direct Reports: Administrative liaison for the Hospitalist Physicians. Maintains effective working relationships with physicians to develop support of departmental programs.

Duty 8: Direct Report Staffing - Selects, leads, coaches, evaluates, develops and recognizes direct reports. Ensures reports embody the BVHS Mission, Vision, and Values and passionately support achieving Pillar accountabilities. Uses positive communication skills, shares information clearly and concisely.

Duty 9: Associate Satisfaction: Accountable for patient, associate, and physician satisfaction related to the medical services provided. Will conduct him/herself in a manner that is supportive of BVHS's commitment to excellence.

Duty 10: Acts in a collaborative fashion with other disciplines and services in the association in order to provide an exceptional continuum of care.

Duty 11: Education: Ensures there are adequate ongoing educational programs and training opportunities for all staff members to enhance the efficiency of operations and provides staff members with opportunities for career and personal growth.

Duty 12: Patient Satisfaction: Accountable for the achievement of upper quartile clinical quality performance for areas of assigned responsibility.

Duty 13: Works with providers in a timely manner to resolve denial/potential denial issues by making phone calls, writing appeal letters, and sending documents to third party payers to avoid penalties or denials of reimbursement.

Duty 14: Communicates effectively with other healthcare disciplines, patients and their families as needed, during a patient's hospitalization so that appropriate levels of care and utilization of services are maintained on an ongoing basis.

REQUIRED QUALIFICATIONS

  • Graduate of an approved school of nursing with a current Ohio license required
  • Expertise in regulatory requirements including ODH, CMS, and Joint Commission for assigned areas
  • Ability to evaluate the quality of care for assigned patients.
  • Ability to establish and maintain appropriate standards of care,
  • Ability to train staff for effective management of patients
  • Ability to effectively coordinate processes for safe and efficient patient disposition, including discharge planning.
  • Positive service-oriented interpersonal and communication skills required.
  • Strong use of spreadsheets, data analytics, program development
  • Ability to positively represent BVHS with Community leaders.
  • A valid driver's license is required (if you do not have a valid Ohio driver's license you must obtain one within 30 days of your residency in the state). You must also meet BVHS's company fleet policy and insurance company requirements, and any other requirements that may be required to operate a vehicle.
  • Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status.Must be able to interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs and to provide the care needed as described in the area's policies and procedures.

PHYSICAL DEMANDS

This position requires a full range of body motion with intermittent walking, lifting, bending, squatting, kneeling, twisting and standing. The associate will be required to walk for up to one hour a day, sit continuously for six hours a day and stand for one hour a day. The individual must be able to lift twenty to fifty pounds and reach work above the shoulders. The individual must have good eye-hand coordination and fine finger dexterity for simple grasping tasks. The individual must have excellent verbal communication skills to perform daily tasks. The associate must have corrected vision and hearing in the normal range. The individual must be able to operate a motor vehicle for business travel and community involvement.

This position is classified at risk for possible occupational exposure to bloodborne pathogens (HBV, HIV, etc.)
